Yarram and District Health Service

At YDHS we offer acute care services including a 24-hour urgent care facility as well as two, 30-bed aged care facilities, including a high-care nursing home and low-care hostel accommodation. Our mission is to excel at providing quality, efficient, people-centred care for our community and to be recognised as a leading rural healthcare service.

YDHS operates two residential aged care facilities. Both facilities are located at our Yarram campus, with immediate access to our acute care, medical centre and allied health services. St Elmo’s is a 30-bed residential aged care facility, which includes dementia-specific capacity, and carer respite while our 30-bed facility, Crossley House, provides residential services as well as carer respite.

A Home Care Package helps you live independently in your own home for as long as you can. The program provides a subsidy towards a package of care services and care management to meet your personal needs. Once allocated to you, a package is available for as long as it meets your level of need or circumstances.

The Yarram Medical Centre is the local GP clinic for residents of Yarram and surrounding areas and is accessible via our main entrance in Commercial Road in Yarram. Our mission at the medical centre is to provide evidence-based patient care incorporating a holistic approach toward diagnosis and management of illness.
